Targeted Case Management

Our skilled Bachelor’s and Master’s level case managers assess consumer needs through a comprehensive Case Management assessment.

Review the consumer’s current medical, behavioral health, housing, employment, educational, and social services needs.

Assist with linkages to resources within the agency and/or in community.

Conduct regular follow up appointments to assess consumer progress and any additional needs that may arise throughout the  treatment.

Continuously work with consumers to address any barriers to treatment.

Provide questionnaires to ensure consumer satisfaction with services and to identify areas for agency improvement.

Consult with various staff members and supervisors as needed to streamline services for consumers.
