Full Job Description

THIS POSITION WILL REQUIRE EVENINGS AND/OR WEEKENDS

  1. Responsible for the overall coordination and monitoring of health assessments for the Expansion and Demonstration programs.
  2. Complete health assessments for Expansion and Demonstration consumers as needed.
  3. Communicate with consumers who cancel or do not show to scheduled health assessments in an effort to identify and address any gaps in services. Work collaboratively with staff to develop a standardized script to be utilized with consumers who are apprehensive about completing a health assessment.
  4. Provide education to consumers on the importance of completing health assessments as part of NECHN integrated model of care.
  5. Reschedule health assessments promptly for the next available appointment or within the required time frames.
  6. Manage spreadsheets for health assessments and 90-day reassessments.
  7. Provide supervision to nursing staff completing health assessments and ensure that nursing staff mark appointments as kept.
  8. Ensure documentation is recorded in EHR in a timely fashion and in accordance with mandated guidelines.
  9. Assist in the data collection process for the Expansion and Demonstration programs with a focus on health assessments and reassessments.
  10. Assist with lab coordination such as: uploading lab documentation, ensure labs are reviewed and signed by physicians/providers, assist consumers with lab related questions or concerns as needed.
  11. Follow up with pre-authorization requests to ensure consumers’ medications are approved by their insurance in a timely manner, in order to minimize delay in the initiation or continuation of treatment.
  12. Document consumer medications received in the medication log and store medication in the medication closet as indicated by the Director of Nursing.
  13. Follow up with consumers lost to contact and not responsive to outreach, and recommend consumers for discharge whenever applicable

THIS POSITION WILL REQUIRE EVENINGS AND/OR WEEKENDS

POSITION SUMMARY:

NECHN is seeking both full and part time Crisis Counselors who can support our team to respond to calls from consumers experiencing mental health-related crises.

In response to each call, the Crisis Counselor will provide crisis stabilization services telephonically or in person with a mobile Crisis Team, depending on the nature of the call. Crisis Counselors will respond to a consumer crisis call within a 3 hour period.

The Crisis Counselor will also support individuals to cope and stabilize after a major crisis, including developing and implementing crisis prevention plans for these consumers. The Crisis Counselor will work with each consumer to ensure each crisis prevention plan includes clear objectives and goals for the consumer in response to their needs, in order to prevent crises from occurring in the future.

The Crisis Counselor will track and document consumer progress in the agency’s Electronic Health Record while working to meet all required internal and external contract standards and policies. The Crisis Counselors will communicate with other agency staff in order to support consistent provision of services.

 

The Crisis Counselor will:

– Respond to crisis calls within 3 hours.

– Provide on call coverage after program hours.

– Coordinate crisis services with their supervisor on and off site, which may require crisis staff to go to a person’s home or to the hospital to work with the screening team and/or emergency squad.

– Provide crisis counseling, as well as individual and group counseling for an assigned caseload of consumers.

– Function as a liaison with DCO Clara Maass Medical Center in cases that require localized Emergency Crisis Screening and/or mobile response.

– Provide follow-up for consumers who are hospitalized and after they return to treatment.

– Utilize conflict resolution strategies in de-escalation and resolving situations.

– Complete crisis assessment of consumers’ substance abuse history (when appropriate) and treatment requirements and develops individualized crisis plan.

– Document all assessments and crisis plans within 24 hours of crisis.

– Attend weekly multi-disciplinary team meetings.

– Determine if a consumer needs hospitalization if a crisis situation is onsite at NECHN. If so, outreach/follow up with the hospital and complete internal incident reports. If not, provide crisis stabilization support, including documenting a crisis progress note and a crisis intervention plan.

– Evaluate, assess, and provide treatment plans to clients utilizing the agency’s urgent care services (Living Room Model).

– Completes Initial Applications and other documentation as assigned.

– Maintain client confidentiality as part of all activities, complying with HIPPA and other privacy regulations.

– Other duties as assigned

 

Qualifications

  • Bachelor’s Degree in Psychology or Related Field (Required)
  • Minimum 1 year of crisis counseling, or similar, experience (Required)
  • Spanish Speaking a plus but not required

EVENINGS AND/OR WEEKENDS HOURS

POSITION SUMMARY:

NECHN is looking for both full and part time clinicians who are able to work within our outpatient substance use disorder program with clients living with co-occurring mental health conditions and substance use disorders. The Clinician will facilitate individual, group, and family counseling. The ideal candidate will be someone who is able to function as an independent healthcare provider who, working collaboratively within a multidisciplinary health team, can provide diagnostic assessment and treatment to support our consumers and their families.

This position will be responsible for the following:

– Engaging consumers in assessments to evaluate their mental health conditions and substance use disorders

– Developing an individualized, integrated treatment plan to meet the holistic needs of each consumer using appropriate diagnostic criteria

– Coordinating client care with other members of the multidisciplinary team, as well as other providers like primary care

– Maintaining progress notes and other documentation required for the client’s medical record

– Using outcome tools to determine patient progress, updating the consumer’s treatment plan as their needs and goals evolve

– Maintaining client confidentiality as part of all activities, complying with HIPPA and other privacy regulations.

 

Qualifications

– Master’s Degree in Social Work or Counseling and NJ State License Required (LSW, LCSW, LAC or LPC)

– Licensed Clinical Social Worker (Preferred)

– Two years of Experience Treating individuals with Substance Use Disorders or Co-Occurring Disorders Required

– LCADC (Preferred)

POSITION SUMMARY:

This position provides Case Management Services to consumers to help them gain access to resources and services needed to meet their mental health needs, achieve recovery, and live independently in the community. Services are coordinated with internal and external providers to help each consumer achieve personalized case management needs.

 

This position will:

– Provide consumers with referrals to services and supports related to housing, applying for welfare, Medicaid enrollment, supportive education, supportive employment, and other needed services.

– Maintain a caseload of case management consumers, for whom the Case Manager coordinates their care.

– Document case management progress notes in a timely fashion.

– Complete all documentation as assigned, including initial applications (phone screens) of individuals who are referred to NECHN and Consumer Advisory Forms.

– Outreach to consumers that have missed their appointments.

– Attend multi-disciplinary team meetings to provide input and obtain feedback about consumer cases.

– Develop community contacts to secure community referrals to ensure consumers needs are met.

– Utilize a person centered approach to provide assistance to consumers to meet their prioritized needs.

– Attend trainings as required, including those related to maintaining positive open and respectful communications with consumers, families and other involved persons.

 

Qualifications

  • BA in human service field
  • At least one year experience in crisis counseling
  • Organized and demonstrates strong leadership and interpersonal skills
  • Ability to work with computers and necessary software

  3. Responsible to collaborate and support consumers to develop individual nutrition, exercise and other healthcare plans and then support persons to follow up on self-identified activities.
  4. Will focus on resiliency and self management, compassion and commitment in helping others
  5. Creates wellness action recovery plans with consumer
  6. Assist persons served to set and achieve health related goals.
  7. Be able to facilitate group and individual sessions focused on WRAP, IMR and Recovery and Wellness.
  8. Assists peers in accessing community resources and natural supports that promote wellness and recovery.
  9. Provide individual and group coaching on wellness activities, i.e., exercise, nutrition, smoking cessation, mind-body spirit connection, WRAP, IMR, etc…
  10. Provides peer support services as part of a multi-disciplinary team.
  11. Identify gaps in care and follows through with consumer eg missed appointments, making sure all intake appointments are made and kept including nursing assessment, case management assessment and SPARS if expansion consumer
  12. Takes minutes during meetings – eg weekly intake meeting as part of staff rotation.
  13. Adheres to HIPPA compliance – eg accesses only consumer records for assigned consumers
  14. Adheres to appropriate boundaries eg contacts with staff and consumers is strictly limited to working hours and related to work of NECHN
  15. Assists consumers in learning about and connect with self help groups and transportation resources.

    • Experience working with WRAP, IMR. CPRR
    • Experience attending or participating in self-help groups or wellness and recovery programs and facilitation.
  • Education:
    • An associate’s degree in psychiatric rehabilitation and two years experience in the provision of mental health services.

The Clinical Supervisor will be responsible for the following:

1. Overseeing and coordinating clinical services within our Outpatient Program.

2. Providing weekly individual and group supervision to assigned clinicians.

3. Providing clinical guidance and support to outpatient team members to help them develop and implement appropriate interventions for individuals living with mental health conditions.

4. Teaching and modeling clinical interventions, recovery-oriented practices, and culturally competent services for each client served.

5. Providing performance evaluations for supervisees.

6. Ensuring supervisees document all services and treatment plans in the EHR in a timely basis.

7. Ensuring appropriate training and utilization of evidence-based practices and the integration of physical, mental health and substance use disorder services.

8. Maintaining accountability for the professional standards within the outpatient clinic and implementing policies and procedures for these areas.

9. Providing backup clinical support to outpatient staff.

10. Maintaining a partial caseload of clients by providing individual and family therapy.

11. Other duties as assigned.

 

Qualifications

– Master’s Degree in Social Work (Required)

– Supervising experience: 2 years (Required)

– Licensed Clinical Social Worker in the state of NJ (Required)

– Certified in Clinical Supervision (20 hour course)

– SIFI (Preferred)

– At least 5 years of experience in the behavioral healthcare field.

– More than 2 years of progressive supervisory experience.

– Familiarity with a wide range of Mental Health issues and DSM diagnoses, including serious and persistent mental illnesses (SPMI).

– Knowledge and understanding of normal childhood development and trauma informed care
